Required Documents
Prepare these items before filling the form.
- Paid SLID voucher reference and voucher application phone number.
- Passport photograph in JPG or PNG format.
- Applicant signature: sign on plain white paper and upload a clear JPG or PNG photo.
- Birth certificate and National Identity Card are mandatory for new applicants.
- Previous passport details for renewals or replacements.
- Police report where lost, stolen, or required.
- Parent or guardian details and ID for child applications.
